When you call for an appointment you will be asked about the nature of your condition so we may schedule you appropriately. Urgent issues will be given priority. Please have your insurance card ready when you call for an appointment. We are open Monday – Friday, from 8am – 5pm, with occasional evening appointments available.
To facilitate your appointment, we kindly ask you to complete the New Patient Form based on your gender (Male or Female). You can find the form under the “Patient Menu” tab on our website or simply click on the following link for the respective form: Male Form or Female Form. Remember to bring the completed form with you to your appointment.
New Patients can typically be scheduled with one of our Board Certified physicians within one week!
If you are given a date for an appointment that is further away than you feel is appropriate for your condition, please ask to speak to a supervisor so that we may assess your needs and schedule you appropriately.
If you have insurance that requires a referral or an authorization, you must have an authorization or a referral number from your primary care physician BEFORE your visit. Our authorization department does its best to request and obtain a referral and/or authorization prior to your visit but ultimately it is your responsibility to have this obtained prior to the visit.
We will make every effort to avoid any inconvenience; however, emergencies do arise as in any busy surgical practice. You will be contacted immediately if a significant delay will affect your appointment. Periodically, we may call patients in the waiting room out of turn if they are seeing us for urgent or brief postoperative examinations. We ask for your patience if you have to wait; we know your time is valuable.
